What is Intracytoplasmic Sperm Injection (ICSI)?

ICSI stands for Intracytoplasmic Sperm Injection, a specialized form of in vitro fertilization (IVF) that is primarily used to treat severe cases of male infertility. Unlike conventional IVF where thousands of sperm are placed near an egg in a petri dish, ICSI involves the direct injection of a single healthy sperm into a mature egg. This increases the chances of successful fertilization, especially in cases where the sperm count is very low or sperm motility is poor.

ICSI is performed in a state-of-the-art embryology lab, and once fertilization occurs, the resulting embryo is transferred into the woman’s uterus for implantation, just like in standard IVF.

    Why is ICSI Done?

ICSI is commonly recommended in the following situations:

    1. Low Sperm Count or Poor Motility – When the sperm are too few or unable to swim efficiently.

    2. Abnormally Shaped Sperm (Teratozoospermia) – Making it difficult for sperm to penetrate the egg naturally.

    3. Previous IVF Failure – When fertilization failed in a prior standard IVF cycle.

    4. Obstructive Azoospermia – Where sperm are not present in the semen but can be surgically retrieved from the testes.

    5. Unexplained Infertility – When no definitive cause is found but fertilization issues are suspected.

    6. Use of Frozen or Surgically Retrieved Sperm – Often less viable than fresh samples.

    7. Antisperm Antibodies – That may affect the sperm’s ability to reach and penetrate the egg.

At SCI IVF Hospital under the expert guidance of Dr. Shivani Sachdev Gour, ICSI is performed with high precision using advanced micromanipulation techniques.

    How to Prepare for ICSI

Preparing for ICSI involves the same foundational steps as IVF but with specific focus on male fertility evaluation. Here’s how the process begins:

    1. Initial Consultation – Meet with a fertility specialist who will review both partners’ medical history.

    2. Semen Analysis – The male partner undergoes a semen analysis to assess sperm count, motility, and morphology.

    3. Sperm Retrieval (if needed) – If sperm is absent in the semen, it can be collected using techniques like TESA (Testicular Sperm Aspiration) or PESA (Percutaneous Epididymal Sperm Aspiration).

    4. Ovarian Reserve Testing (for the female) – Blood tests like AMH and ultrasound for antral follicle count to assess egg supply.

    5. Infection Screening – Both partners may be tested for infections or conditions that could affect fertility.

Choosing a reputed clinic like SCI IVF Hospital in Delhi NCR ensures access to skilled embryologists, latest ICSI technologies, and personalized care.

    ICSI Procedure Step By Step

Just like IVF, ICSI is a multi-step process involving hormonal stimulation, egg retrieval, fertilization, and embryo transfer. The difference lies in the fertilization technique.

    1. Ovarian Stimulation

The female partner is given hormonal injections to stimulate the ovaries to produce multiple eggs. This step is carefully monitored using ultrasound and blood hormone levels.

    2. Egg Retrieval

Once the eggs are mature, they are retrieved under sedation using a transvaginal ultrasound-guided needle. This process is performed under anesthesia and usually takes about 20-30 minutes.

    3. Sperm Collection or Retrieval

The male partner provides a semen sample, or sperm is extracted surgically in case of azoospermia.

    4. Sperm Injection (ICSI)

This is the key step that differentiates ICSI from IVF. A single motile and healthy sperm is selected and injected directly into the cytoplasm of each mature egg using a microinjection needle under a high-powered microscope.

    5. Embryo Culture

The fertilized eggs are monitored for a few days (3 to 5) to check for proper cell division and development into blastocysts.

    6. Embryo Transfer

The best quality embryo(s) are selected and transferred into the uterus. The number of embryos transferred is decided based on the woman’s age, embryo quality, and medical history.

    7. Pregnancy Support

Post-transfer, progesterone support is provided to help the uterus lining support implantation. After about 10-14 days, a blood test (beta-hCG) is conducted to confirm pregnancy.

    Risks and Complications of ICSI

Although ICSI is a safe and effective procedure, it carries some risks:

Fertilization Failure – Despite injecting sperm directly, fertilization might not occur.

Embryo Development Issues – Not all fertilized eggs will develop into healthy embryos.

Birth Defects (very rare) – Slightly higher risk than natural conception, mostly related to underlying sperm quality.

Ovarian Hyperstimulation Syndrome (OHSS) – Due to fertility medications in female partners.

Multiple Pregnancy – If more than one embryo is transferred, it increases the chance of twins or triplets.

    ICSI Results

Success rates for ICSI largely depend on factors such as:

    • Age of the female partner.
    • Quality of eggs and sperm.
    • Expertise of the IVF lab and embryologist.

At SCI IVF Hospital, the success rate of ICSI is significantly high due to advanced lab facilities and the expert hands of embryologists led by Dr. Shivani Sachdev Gour.

After embryo transfer, pregnancy results are confirmed through blood tests. If the result is positive, the woman will be guided through her early pregnancy phase with proper monitoring and prenatal care. If the cycle is unsuccessful, couples are counseled and further treatment options are discussed.
