What is In Vitro Fertilization (IVF)?

In Vitro Fertilization (IVF) is one of the most advanced and effective assisted reproductive technologies (ART) available today. The term “In Vitro” means “outside the body,” and IVF refers to the process in which an egg is fertilized by sperm in a laboratory environment rather than inside a woman’s body.

This process is a ray of hope for couples who have struggled with infertility and failed to conceive through natural methods or other simpler fertility treatments. IVF involves a sequence of procedures, starting from ovarian stimulation and egg retrieval, followed by fertilization, embryo culture, and finally embryo transfer into the uterus. If successful, the embryo implants in the uterine wall and leads to pregnancy.

At SCI IVF Hospitals, under the expert leadership of Dr. Shivani Sachdev Gour, IVF has helped thousands of couples realize their dream of parenthood. Dr. Shivani brings years of global experience, having trained and practiced in the UK and India, and is recognized as one of the pioneers in the field of infertility treatment.

    Why is IVF Done?

IVF is typically recommended when other fertility treatments such as ovulation induction medications or intrauterine insemination (IUI) have not been successful. It is considered the best approach in various complex cases of infertility:

    Common Indications for IVF:

    1. Age-related infertility in women, especially after 35 years, when natural fertility declines.
    2. Decreased ovarian reserve, meaning low quantity and/or poor quality of eggs.
    3. Tubal factor infertility, such as damaged or blocked fallopian tubes.
    4. Male factor infertility, including low sperm count, motility, or abnormal morphology.
    5. Endometriosis, where the uterine lining grows outside the uterus affecting fertility.
    6. Polycystic Ovary Syndrome (PCOS), especially when not responding to medications.
    7. Uterine issues, such as fibroids or congenital abnormalities.
    8. Unexplained infertility, when the exact cause of infertility cannot be identified.
    9. Genetic conditions, requiring preimplantation genetic testing (PGT).
    10. Repeated miscarriages or failed IUI attempts.
    11. Fertility preservation before cancer treatment or for social reasons (egg/sperm freezing).

    How to Prepare for IVF

Preparation is a critical phase of the IVF journey. Once you decide to seek help from a fertility expert, choosing the right IVF center is the first and most important step. SCI IVF Hospitals, headed by Dr. Shivani Sachdev Gour, stand out for their transparent treatment protocols, advanced lab infrastructure, and high success rates.

    Initial Fertility Assessments Include:

    For Women:

    1. Ovarian Reserve Testing: AMH (Anti-Müllerian Hormone) blood test and Antral Follicle Count (AFC) via transvaginal ultrasound to assess egg quantity.
    2. Pelvic Ultrasound: To evaluate uterine structure, endometrial lining, and detect conditions like fibroids or cysts.
    3. Hormonal Profile: Including FSH, LH, TSH, Prolactin, Estradiol, and others.
    4. Infection Screening: To rule out sexually transmitted infections or other reproductive tract infections.

    For Men:

    1. Semen Analysis: To assess sperm count, motility, morphology, and volume.
    2. Additional Tests (if needed): Hormone tests, genetic testing, or testicular biopsy in case of azoospermia.

Each case is discussed in detail by Dr. Shivani Sachdev Gour and her team, ensuring a personalized and compassionate approach from the start.

    IVF Procedure Step-by-Step

    1. Ovarian Stimulation
During a natural cycle, a woman produces only one mature egg. However, IVF requires multiple eggs to increase the chances of success. You will be given hormonal injections (FSH, HMG) for 8–14 days to stimulate your ovaries to produce multiple follicles. The growth of these follicles is closely monitored through serial ultrasounds and blood tests. Once the eggs are mature, a trigger injection (hCG) is given.

    2. Egg Retrieval (Ovum Pick-Up)

34–36 hours after the trigger injection, egg retrieval is scheduled. The procedure is performed under mild anesthesia. Using a fine needle guided by ultrasound, the doctor aspirates the fluid from each follicle and collects the eggs.

    3. Sperm Collection and Processing

On the same day, the male partner is asked to provide a semen sample. The sperm is then washed and prepared in the lab to select the healthiest and most motile sperms for fertilization.

    4. Fertilization

There are two methods of fertilization:

    • Conventional IVF: The eggs and sperms are combined in a petri dish and allowed to fertilize naturally.
    • ICSI (Intracytoplasmic Sperm Injection): A single healthy sperm is directly injected into the egg. This method is used in cases of male infertility or failed fertilization in previous cycles.

    5. Embryo Culture

Fertilized eggs (embryos) are cultured in a controlled environment. Dr. Shivani closely monitors embryo development from Day 1 to Day 5 (blastocyst stage), choosing only the best embryos for transfer.

    6. Embryo Transfer

The healthiest embryo(s) are selected and transferred into the woman’s uterus using a thin catheter. This is a painless, short procedure done without anesthesia. The number of embryos transferred depends on the woman’s age and embryo quality, keeping in mind the risk of multiple pregnancies. Dr. Shivani Sachdev Gour personally performs most embryo transfers, ensuring precision and care.

    7. Pregnancy Support and Implantation

Post-transfer, hormonal support (mainly progesterone injections or tablets) is provided to help with the implantation process and early pregnancy. Implantation usually occurs within 5–10 days after the transfer.

    Risks and Complications of IVF

Though IVF is safe, like all medical procedures, it comes with certain risks:

    1. Multiple Pregnancies: May lead to preterm labor and low birth weight.
    2. Ovarian Hyperstimulation Syndrome (OHSS): Overreaction to fertility medications.
    3. Ectopic Pregnancy: Implantation outside the uterus (in fallopian tubes).
    4. Miscarriage: Risk similar to natural pregnancy (~15–20%), increases with age.
    5. Emotional Stress: The financial, physical, and emotional demands can be overwhelming.
    6. Procedure-Related Complications: Rare, but include bleeding or infection after egg retrieval.

Dr. Shivani Sachdev Gour ensures patients are fully informed of risks and helps them make confident decisions throughout the process.

    IVF Results and Success Rates

A beta-hCG blood test is done approximately 14 days after embryo transfer to confirm pregnancy. Based on the outcome:

    1. Positive Result: Indicates pregnancy; ultrasound is scheduled after 2–3 weeks to detect heartbeat.
    2. Negative Result: Medications are stopped and periods may resume naturally. Your doctor will guide you on next steps, which may include another cycle.

Success varies depending on age, egg quality, sperm health, and clinic expertise. While some couples succeed in the first cycle, others may need multiple attempts. At SCI IVF Hospital, under Dr. Shivani Sachdev Gour’s guidance, the success rates are among the highest in India — especially in women under 35 years of age.
